To qualify for assistance from this fund, applicants must meet all of the following criteria.

The patient has been diagnosed with Lung Cancer within the last 3 years unless they reside in: Maine, Minnesota, North Dakota - 6 Years; Missouri - 5 Years; Nebraska, Utah, Wyoming - 4 Years

The patient has a biopsy/pathology report confirming the diagnosis.* Please submit report when you submit your request.

The patient lives within the continental United States.

Completed Cinnamon Helps Patient Authorization form.* Please submit authorization form when you submit your request.
