Eligibility Requirements

To qualify for assistance from this fund, applicants must meet all of the following criteria.

1
The patient was a firefighter after 1960 for at least 4 years and was exposed to AFFF (includes civilian firefighters) OR
2
The patient was in the military or at least 4 years and was exposed to AFFF (Aqueous Film Forming Foam) OR
3
The patient was exposed to AFFF at least 10 times at work within 4 years. OR
4
I would like to be contacted to see if I match geographic criteria (for patients who do not meet any of the first two options)
5
Completed Cinnamon Helps Patient Authorization form. (Please submit authorization form when you submit your request.)
