Patient must be 21 years old or older.

Patient must have been diagnosed with cancer within the last 3 years.

Patient must be U.S. Citizen.
4
Patient must be receiving treatment from a U.S. healthcare provider.
5
Patients must have an outstanding balance at their treating medical facility.
